Prior Approval Continuing Education

If you are interested in applying for Continuing Education credits please complete the form.  Only online submissions will be accepted.   Please have your submission completed ans submitted four weeks prior to your event.  

Please note if your event is scheduled within two weeks of today's date, please contact for approval to proceed with your submission. 

There is a $10 fee per credit that must be submitted with this form.  If you are applying for credits within 2 weeks of your event, please be aware that a rush fee of $20 a credit must be submitted.

In order for your submission to be considered we must receive all of the following documentation:

  • Educational Objectives

  • Learning Codes 

  • Timing Outline

  • Target Audience

  • Qualifications of Speakers
